Managing Your Healthcare With Vascuitis

One of the more challenging aspects of managing daily life with vasculitis is keeping up with medications and medical information. This is important for all aspects of your health, but this is particularly important when you have vasculitis.

It is crucial that you and your doctors know exactly what medications you are taking, the dosage, and when you are taking them.

Often patients with vasculitis will have multiple physicians providing care for them as well. It is important that there is good communication between all providers. We strive for this in the Johns Hopkins Vasculitis Center.

Below are two tools which will help you keep track of your medications and health care providers. Keep these lists accurate and bring them to every doctor’s appointment. It will allow for all of your providers to stay informed and have the most accurate information to provide you with exceptional care.

  • Medications Log is a form is designed to help you keep track of what you should take, what you did take and when. Click on either the Microsoft Word icon or the Adobe PDF icon open and save the document to your computer. (Word, PDF)
  • Medical Contact for Physicians, Pharmacies, and Hospitals puts all your health care contact information in one place. Click on either the Microsoft Word icon or the Adobe PDF icon open and save the document to your computer. (Word, PDF)
